Diagnosis of Sinusitis

Diagnosis of sinusitis involves a series of blood and imaging tests. These include:

Routine blood tests. These help in detection of an infection. This infection may lead to increase in the white blood cell counts. Allergies or hay fever giving rise to sinusitis may cause rise in specific type of white blood cells called the Eosinophil.

Since persons with HIV AIDS, diabetes and other poor immunity situations are more likely to get sinusitis, blood tests for these may be recommended.

Transillumination test: The ENT specialist doctor uses a special light to examine the inside of the nose. This helps in detection of nasal polyps, nasal bony spurs, deviated nasal septum and other anatomical defects that may lead to narrowing of nasal passages and predispose to sinusitis. A light is shined against the sinuses. Normally the sinus appears hollow and the light shines through, giving a reddish glow. When inflamed and blocked with secretions and mucus, the light fails to shine through, and the sinus appears opaque.

X-ray: Normal sinuses show up as hollow black cavities on either side of the forehead, bridge of the nose, behind the yes and under the cheek bones. When inflamed, the sinuses appear to be blocked with white discharge and this is visible on X-rays.

Nasal Endoscopy: A new method of inspecting the insides of the sinuses is the fibreoptic endoscope or the rhino scope. This is a thin flexible tube with a camera and a light at its tip. The nasal passages are lubricated with local anaesthetics and the scope is passed. The inside walls and linings of the sinuses may be visualized with this instrument.

CT scan or MRI scan: When fungal sinusitis or sinus tumours are suspected or need to be ruled out imaging studies like ultrasonography of the sinuses, CT scan or MRI scan of the sinuses may be prescribed. These also help detect anatomical abnormalities of the nose and sinuses.

Allergy testing: Those with allergies may be recommended allergy testing to detect the cause of allergies.

Sweat Chloride tests: Certain tests are available to detect if the ciliary cells of the nose and sinuses are functioning adequately. Sweat Chloride tests are ordered to exclude Cystic fibrosis that predisposes to sinusitis.

Cells of the nasal and sinus lining are taken as samples and examined under the microscope for abnormalities. Conditions to be ruled out in diagnosing sinusitis (because they mimic symptoms of sinusitis) include allergic rhinitis, common cold, adenoiditis in children and other causes of headaches.

Treatment for Mild and Chronic Sinusitis

Sinusitis is treated in several ways, each depending on how severe the case of sinusitis is.

A simple sinusitis infection is treated with:

  • Decongestants
  • Cold and allergy medications
  • Nasal saline irrigation
  • Drinking fluids (sinusitis is a viral infection and fluids will help)

If symptoms of sinusitis don't improve after 10 days, ENT doctor may prescribe:

  • Antibiotics (for seven days in adults and 10 days in children)
  • Oral or topical decongestants
  • Prescription intranasal steroid sprays. (Don't use non-prescription sprays or drops for longer than three to five days — they may actually increase congestion)

Long-term (chronic) sinusitis may be treated by focusing on the underlying condition (typically allergies). This is usually treated with:

  • Intranasal steroid sprays
  • Topical antihistamine sprays or oral pills
  • Leukotriene antagonists to reduce swelling and allergy symptoms
  • Rinsing the nose with saline solutions that might also contain other types of medication

When sinusitis isn't controlled by one of the above treatments, a CT scan is used to take a better look at your sinuses. Depending on the results, surgery may be needed to correct structural problems in your sinuses. This is most likely to happen if you have polyps and/or a fungal infection.

Sinus Surgery for Sinusitis Treatment

Functional endoscopic sinus surgery FESS in Hyderabad, Telangana, India

Functional Endoscopic Sinus Surgery (FESS)

One of today's most common sinus surgery options, FESS is a procedure that utilizes a magnifying endoscope to both view the sinuses and remove any affected tissue or bone. It's reconstructive to open the connection between the sinuses and nose. The result should produce better drainage, reduce blockages and flush out infected material while maintaining healthy tissue for the nose to operate normally. As the name suggests, the procedure is endoscopic. ENT surgeon makes small incisions inside the nose to allow for a small tube with a camera and light at the end (endoscope) to survey the inside of the nasal cavity. FESS is often performed in conjunction with other sinus surgery options like septoplasty.

Balloon Sinuplasty Sinus Surgery in Hyderabad, Telangana, India

Balloon Sinuplasty

Balloon Sinuplasty is a minimal invasive procedure designed to treat patients and give relief from chronic sinusitis symptoms. The method is a straightforward application wherein the doctor inserts a flexible balloon catheter into the sinus passage. The process is aided by a tiny wire with a flashlight on the end to help navigation. Once the balloon is appropriately positioned, it's slowly inflated to open up the sinus cavity. The ENT doctor uses a saline solution to help flush out the mucus and pus. Many patients feel an instant relief in pressure once the fluid starts to drain. The gentle pressure of the balloon against the sinus cavity helps to restructure the bones and cartilage. After its removal, the sinuses should feel clearer.
