Liver cancer Diagnosis

While there are various liver cancer diagnostic tests to identify different types of liver cancer, not every individual necessarily needs to undergo every test outlined here. When selecting a diagnostic test, the oncologist may consider the following:

  • Suspected kind of cancer
  • Symptoms and indicators
  • Age and health condition
  • The outcomes of previous medical exams


Usually, with noninvasive imaging, the diagnosis can be confirmed. Even when biopsy is needed, imaging is usually required for guidance. Before prescribing any laboratory tests or scans, the liver cancer specialist doctor looks for signs of liver cancer on the body of the patient. The doctor may perform the following tasks, such as:

To confirm the diagnosis of liver cancer, the liver cancer specialist doctor may prescribe various other laboratory tests or scans, such as: 


Blood tests: In conjunction with the physical examination, the physician will usually perform a blood test to detect alpha-fetoprotein (AFP). 

  • Approximately 50-70% of individuals with liver cancer in have increased levels of AFP in their blood. 
  • The doctor will also test the individual's blood for hepatitis B or C. 
  • Other blood tests may indicate the health of the liver.
  • Usually, an increase of AFP above 500 mcg/L could be an indication of liver cancer.

  • AFP cannot be used to assess therapy response, however, if its levels were not high before treatment. 
  • Approximately one-third of liver cancer patients do not have an elevated AFP level in liver cancer.

Ultrasonography: provides image of the body's internal components through ultrasound. 

  • The sound waves reflect off the liver and other organs, as well as malignancies. 
  • Each reflected wave produces a unique image on a computer display.

Computed tomography (CT or CAT) scan: Using x-rays gathered from many angles, a CT scan generates a three-dimensional representation of the body's interior.

  • The patient is anaesthetized as the tube is implanted via a minor abdominal incision. 
  • Laparoscopy is rarely used to diagnose liver cancer.

Liver Biopsy: the removal of a tiny sample of tissue from liver for microscopic examination and then analysed by a pathologist. 

Biomarker testing of the tumour (also called as tumour molecular testing): Laboratory tests for determining liver cancer by taking blood and analysing it. Biomarker testing also determines the treatment choices including targeted therapy.

Liver cancer staging system

There have been a number of staging systems developed for the purpose of determining the prognosis of liver cancer, such as: 

  • Tumour-node metastasis (TNM) (the most widely used of these staging systems)

  • American Joint Committee on Cancer (AJCC)
  • Barcelona Clinic Liver Cancer (BCLC) systems, and

The Barcelona Clinic Liver Cancer (BCLC) method is often used by medical professionals to understand the stage of liver cancer through which the treatment choices can be managed and manipulated. The BCLC method classifies liver cancer according to the features of the tumour, as well as liver function, performance status, and symptoms associated with malignancy.


The BCLC stage groupings include:

Very early stage: is defined if the size of the tumour is less than two centimetres. 

  • There is no indication of a rise in pressure in the portal vein, (one of the primary veins that carry blood out from the liver).

  • Clinical staging systems, such as the BCLC and CLIP systems, may have more prognostic value for patients with advanced liver cancer and cirrhosis who are not candidates for surgery. 
  • On the other hand, pathologic staging systems, such as the AJCC and TNM staging system, may be superior to clinical staging systems in prognosis classification for patients who have surgical resection.

Liver cancer differential diagnosis

  • Dysplastic nodules in cirrhosis (nodules with a diameter of 1 mm or more, show abnormal tissue development, and lack definite histopathologic findings of malignancy).
  • Fibrous nodular hyperplasia (the most frequent benign, non-neoplastic, reactive growth of the liver).
  • Primary hepatic lymphoma (rare type of primary liver tumour and an unusual type of lymphoma, usually presenting with constitutional symptoms such as: 
  • hepatomegaly but without lymphadenopathy and 
  • extrahepatic lymphomas i.e., bone marrow, other lymphoid tissue, spleen).
  • Cholangiocarcinoma (bile duct cancer).
  • Cirrhosis (advanced liver-stage disease where the healthy liver tissue is replaced with scar tissue, permanently damaging the liver).
  • Hepatocellular Adenoma (Hepatic Adenoma) (rare, but benign epithelial tumours of the liver frequently associated with oral contraceptive pill use).

The goals of a multidisciplinary team before treating a liver cancer


Each type of treatment has a different goal. The goal of liver cancer treatment is to do one or more of these things:

  • Remove the cancer in the liver (or the entire liver) while doing as little damage as possible to nearby areas
  • Kill cancer cells or keep them from growing or spreading
  • Keep the cancer from coming back or delay its return
  • Ease symptoms of the cancer, such as pain or blockages

Treatments which can potentially cure the cancer 


Surgery for liver cancer: Surgery is the removal of the tumour and some healthy tissue during operation. 

  • It is expected to be the most effective disease-directed therapy, especially for individuals with strong liver function and few liver tumours. 
  • If the tumour is too large, the liver is so damaged, the tumour has spread, or the patient has other significant conditions, surgery may not be an option.

Two types of surgery are used to treat liver cancer - hepatectomy and liver transplantation.

  • Hepatectomy: a portion of the liver is removed. A hepatectomy is only possible if the liver is healthy and the malignancy is in one portion. The remaining liver segment performs all tasks. The liver may recover within weeks. Even if the tumour is tiny, cirrhosis may prevent a hepatectomy. The side effects include pain, weakness, fatigue, and temporary liver failure. 
  • Liver transplantation: As the name suggests, this operation replaces a patient's liver with healthy donor tissue. This treatment is only possible with specified tumour size, number, and donor. These criteria include a single ≤ 5-cm tumour or ≤ 3 tumours. Transplantation isn't always an option due to the limited liver donor tissues.


After a liver transplant, the patient is continuously monitored for any organ rejection or tumour regrowth. Medication must be administered to prevent rejection. These medicines may induce various side effects such as facial puffiness, high blood pressure, and body hair growth. Liver transplants include hazards of infection, organ rejection, and other malignancies.


Radiofrequency ablation (RFA, also called thermal ablation)

  • RFA and microwave treatment kill cancer cells with heat. They may be delivered via the skin, laparoscopy, or during a sedated surgery. The patient is sedated to help relax during the procedure. 
  • Percutaneous ethanol injection is done directly into the liver tumour to destroy it. The process causes fever and pain. A simple, safe, and successful for tumours under 3 cm. If alcohol exits the liver, severe pain follows. 


Radiation therapy

  • The radiation therapy destroys cancer cells using high-energy x-rays or particles. A radiation therapy programme involves a defined number of treatment sessions over a period of time. 
  • Stereotactic body radiation therapy (SBRT) is a type of radiation therapy in which high doses of radiation are delivered to a hepatic tumour tissue to destroy it while limiting radiation to nearby healthy tissue. 
  • SBRT effectively targets 5-cm-or-smaller tumours. SBRT may cause adverse effects to stomach and lung, but can be usually avoided. 


Treatments which can potentially increase the patient's chances of surviving the illness but will not, in all likelihood, get rid of the cancer. 


Chemoembolization and radioembolization for disease confined to the liver

  • Chemoembolization – this procedure utilises concentrated chemotherapeutic agent with ethiodized oil. Chemotherapy uses chemicals to kill cancer cells by preventing them from dividing and multiplying. During this operation, injections into the hepatic artery temporarily blocks the blood, enhancing the duration of chemotherapy and killing the cancer cells. Apart from primary treatment for liver cancer, chemoembolization can “slow” tumour growth for people waiting for liver transplantation. 
  • Radioembolization - radioactive beads are placed into the artery that supplies the tumour with blood. The beads deliver radiation therapy directly into the tumour when they are nearing the tumour. 
  • Systemic therapy for advanced HCC includes medication administered through the bloodstream may reach cancer cells throughout the body. Local treatment involves applying medicine directly to the cancer or to a specific bodily component. This therapy is usually recommended by a medical oncologist. Medications are generally delivered by an intravenous tube, a muscle or subcutaneous injection, or a tablet or capsule (orally). Treatment for advanced liver cancer using Systemic Therapy


Standard Frontline Therapy

  • Absolute contraindications for TACE are decompensated cirrhosis and severe reduced hepatic portal blood flow

Radioembolization
